The Tennessee Department of Health hereinafter referred to as "State” or "Department" is soliciting grant proposals for the administration and oversight of a community grant program to support the implementation of the Tennessee Maternal Health Strategic Plan (MHSP).

The purpose of this Request for Grant Proposal (RFGP) is to define the State's minimum requirements, solicit grant proposals and gain adequate information from which the State can evaluate the services you propose to provide. The State intends to enter into/award up to one (1) grant for a period of fifteen (15) months with an expected effective period from July 1, 2026 to September 30, 2027 and with a maximum grant amount of $300,000.00.

The selected Grantee will match $300,000 in philanthropic funds, held in a designated maternal health depository, to expand the total available funding for project implementation.

The combined funds will support the design and delivery of at least two (2) initiatives that align with one or more of the following MHSP focus areas:

  • Increase access to quality maternal healthcare for pregnant and postpartum individuals in Tennessee.
  • Strengthen maternal health infrastructure and systems at the community or regional level.
  • Expand and support the maternal health workforce, including birth workers, doulas, and clinical providers.
  • Address key influences on maternal health outcomes, including transportation, housing, mental health, and social support needs.

All initiatives must be carried out in partnership with local organizations and must respond to clearly identified maternal health needs, with a focus on underserved, rural, or high-risk populations. The Grantee shall maintain a financial tracking system to document both grant expenditure and matching funds. Quarterly fiscal and programmatic reports will be required. All activities and final deliverables must be completed no later than September 30, 2027.
